liaison<\/p><p>The Hearth Relationship Manager:<\/p><ul><li>Establishes a relationship with the Client\/Participant to understand the Participants goals, interests, and support needs<\/li><li>Works collaboratively with the Client\/Participant, and support team to bring the high-level <a href=\"https:\/\/www.ndis.gov.au\/participants\/how-planning-process-works\">NDIS Participant Plan<\/a> goals to life<\/li><li>Ensures everybody is working together as a collaborative team to meet the support needs of the Client\/Participant.<\/li><li>Is a resource provided at no cost to the Client\/Participant \u2013 Hearth\u2019s investment in the Client\/Participant experience<\/li><li>The Relationship manager is supported and enabled by a team of experts \u2013 across Human Resources, Recruitment, Compliance, Finance, Marketing &amp; Initially up-front qualifications and or experience is welcome but not essential.
