Support Worker matching<\/strong><\/span><\/p><p><em><strong>Step 1<\/strong><\/em><\/p><ul><li>Establishes a relationship with the Client\/Participant to understand the Participants goals, interests, and support needs<\/li><li>Before matching takes into consideration Hearth Participants<ul><li>Vary enormously by personality, goals, interests, age, and disability.<\/li><li>Every Participant and family situation is unique<\/li><\/ul><\/li><\/ul><p><em><strong>Step 2<\/strong><\/em><\/p><ul><li>Looks through the Hearth talent pool to find a suitable potential Disability Support Worker who matches the Participants interests and is available when required<\/li><li>Collaborates with the Client\/Participant to build a long-term sustainable support team, nurturing a partnership with the right Support Worker\/s who really know the Participants goals, interests, and passions<\/li><li>Ensures the foundations of support are the right human connection.<\/li><\/ul>\t\t\t\t\t\t<\/div>\n\t\t\t\t<\/div>\n\t\t\t\t\t<\/div>\n\t\t<\/div>\n\t\t\t\t<div class=\"elementor-column elementor-col-50 elementor-inner-column elementor-element elementor-element-ffa4447\" data-id=\"ffa4447\" data-element_type=\"column\">\n\t\t\t<div class=\"elementor-widget-wrap elementor-element-populated\">\n\t\t\t\t\t\t<div class=\"elementor-element elementor-element-d228b09 elementor-widget elementor-widget-text-editor\" data-id=\"d228b09\" data-element_type=\"widget\" data-widget_type=\"text-editor.default\">\n\t\t\t\t<div class=\"elementor-widget-container\">\n\t\t\t\t\t\t\t<ul><li><strong>Organises and conducts the initial Support Worker introduction to the Client\/Participant\u2019s in the Client\/Participants home<\/strong><\/li><li>Consults with the Client\/Participant\/Support Worker to ensure all parties are comfortable with the proposed match<\/li><\/ul><p><em><strong>Step 3<\/strong><\/em><\/p><ul><li>Once the relationship and match is established, then coordinates the training to equip the Support Worker\/s with <strong>the necessary training<\/strong> to correctly support the Participant<\/li><li>If the Client has more challenging or complex support needs the Hearth Relationship Manager can also organize <strong>\u201cshadow shifts.\u201d<\/strong> Shadow shifts are organised to assist new Disability Support Workers to learn what is required to support the Client from a Disability Support Worker who has previously or currently supports the Client.<\/li><li>Develops a support team of experienced Support Workers over time initially <strong>up-front qualifications and or experience are welcome but not essential the right human connection is essential<\/strong><\/li><\/ul>\t\t\t\t\t\t<\/div>\n\t\t\t\t<\/div>\n\t\t\t\t\t<\/div>\n\t\t<\/div>\n\t\t\t\t\t<\/div>\n\t\t<\/section>\n\t\t\t\t<section class=\"elementor-section elementor-inner-section elementor-element elementor-element-4ec9cbe elementor-section-boxed elementor-section-height-default elementor-section-height-default\" data-id=\"4ec9cbe\" data-element_type=\"section\">\n\t\t\t\t\t\t<div class=\"elementor-container elementor-column-gap-default\">\n\t\t\t\t\t<div class=\"elementor-column elementor-col-100 elementor-inner-column elementor-element elementor-element-7376b5c\" data-id=\"7376b5c\" data-element_type=\"column\" data-settings=\"{&quot;background_background&quot;:&quot;classic&quot;}\">\n\t\t\t<div class=\"elementor-widget-wrap elementor-element-populated\">\n\t\t\t\t\t\t<div class=\"elementor-element elementor-element-cf39dd4 elementor-widget elementor-widget-heading\" data-id=\"cf39dd4\" data-element_type=\"widget\" data-widget_type=\"heading.default\">\n\t\t\t\t<div class=\"elementor-widget-container\">\n\t\t\t<h5 class=\"elementor-heading-title elementor-size-default\">Every Participant and family situation is unique<\/h5>\t\t<\/div>\n\t\t\t\t<\/div>\n\t\t\t\t\t<\/div>\n\t\t<\/div>\n\t\t\t\t\t<\/div>\n\t\t<\/section>\n\t\t\t\t\t<\/div>\n\t\t<\/div>\n\t\t\t\t\t<\/div>\n\t\t<\/section>\n\t\t\t\t<\/div>\n\t\t","protected":false},"excerpt":{"rendered":"<p>The Hearth Participant &amp;
